What is a Support Plan?
This is a list of things that are important to you and that you need to meet your outcomes.
It should look at your whole life, what works for you and what doesn't and what could be improved.
It will include the outcomes you wish to achieve, how you wish to achieve them and what they will cost.
Things to think about include...
q What is important to me?
q What make me happy?
q What do I want to stay the same?
q What do I want to change?
q How will I get the support I need?
q How will my Personal Budget be spent?
q How can I look after my Personal Budget?
q When can I make all this happen?
Who will help me complete my Support Plan?
You can complete this with the help of friends, family, your Care Manager or with the help of an advocate or brokerage service. Your Support Plan can be as detailed or simple as you want it to be.
What happens next?
When your Support Plan is agreed by a social care professional, you will then be told how much you will receive as your Personal Budget. You will also be told whether you will need to contribute towards your care costs.
